New Patient Paperwork
Please arrive 15 minutes prior to your scheduled appointment with a photo ID and insurance card. Our staff will request that you fill out all the forms attached below. To save time, feel free to print and fill out the forms before you arrive, otherwise we will provide them for you.
Please note that we reserve the right to charge a fee for any scheduled appointments that are either missed or canceled within less than 24-hour notice.

Insurance
At the Center for Glaucoma & Cataract Care, we are Medicare partners and accept most major insurance plans. If you have any questions regarding your specific plan we recommend contacting your insurance company directly.
Services not covered by insurance are the patient’s responsibility
We do not participate with any vision plans
We do not accept Medicaid with the exception of UnitedHealthcare Community Care
Our Office Hours
Monday – Thursday: 8:00 AM to 5:30 PM
Friday: 8:00 AM to 12:00 PM
If you have an eye emergency after business hours, please visit your local emergency room.
