First Time Appointments

Please arrive 15 minutes prior to your scheduled appointment with a photo ID and insurance card. Our staff will request that you fill out all the forms attached below. To save time, feel free to print and fill out the forms before you arrive, otherwise we will provide them for you.

Insurance

At the Center for Glaucoma & Cataract Care, we are medicare partners and accept most major insurance plans. If you have any questions regarding your specific plan we recommend contacting your insurance company directly.

  • Feel free to call and ask about your particular insurance plan

  • Services not covered by insurance are the patient’s responsibility

  • At this time, we do not participate with any vision plans

Monday – Thursday: 8:00 AM to 5:30 PM
Friday: 8:00 AM to 12:00 PM

If you have an eye emergency after business hours, please visit your local emergency room.
